This website is operated by Sanare Integrated Health Services, National Highway, Barangay Kalawag II, Isulan, Sultan Kudarat, Philippines. “We” and “us” mean Sanare; “you” means the person using this website. If you do not accept these terms, please do not use the website — you are of course still welcome to visit the clinic in person.

The content on this website — service descriptions, the animal bite dose schedule, the answers to frequently asked questions, and everything else — is general information about what we offer. It is not medical advice, it is not a diagnosis, and it is not a substitute for being examined by a clinician.

Never delay seeking care, and never stop or change a prescribed medicine, on the basis of something you read here. Advice specific to you can only come from a clinician who has assessed you.

• A booking request is confirmed only once we confirm it. A slot shown as available may already have been taken by another patient while you were completing the form.
• Please arrive at least fifteen (15) minutes before your appointment time to check in. Arriving late may mean your slot is given to a waiting patient and you are served in the walk-in queue instead.
• If you cannot attend, please cancel or reschedule in the patient portal, or call us. Repeated failures to attend without notice may lead us to limit online booking on your account.
• Queue numbers issued on registration are served in order of clinical priority first and arrival time second. A patient in a more urgent condition will be seen before you, even if they arrived later.
• Doctor schedules published here are the regular weekly hours. They may change without notice because of holidays, leave or emergencies.
• We may have to cancel or reschedule an appointment for reasons beyond our control. Where we do, we will contact you using the details on your record.

• Your account is for you alone. Keep your password and any one-time codes confidential, and do not let anyone else use your account.
• Tell us immediately if you believe someone else has accessed your account.
• The information you give us must be accurate, complete and current. Registering under another person’s identity, or giving false health information, endangers you and may render treatment unsafe.
• You may manage a dependent’s record only where we have recorded your authority to do so.
• We may suspend or close an account that is used to impersonate someone, to submit false information, or to interfere with our systems or with other patients’ care.

You agree not to:

• attempt to access any record, account or part of our systems that is not yours;
• probe, scan, overload or otherwise interfere with the operation or security of this website;
• make bookings or registrations in bad faith, in bulk, or by automated means;
• copy, scrape or republish content from this website for commercial purposes without our written permission; or
• use this website for any unlawful purpose.

Fees shown on this website are indicative starting fees for consultation or treatment. They do not include medicines, vaccines, supplies, procedures or laboratory referrals, and they may change. The amount payable is the amount on the bill issued to you at the clinic.

Senior citizens and persons with disability receive the statutory discount and VAT exemption on eligible charges, on presentation of a valid OSCA or PWD identification card. Where you are claiming a PhilHealth benefit, our billing staff will tell you what is covered before you pay; charges not covered by PhilHealth are payable by you. No payment is taken through this website.

You are free to have your prescription filled at any pharmacy you choose. Our pharmacy is offered for your convenience only, and your care is not affected by where you buy your medicines.

How we collect, use, share, protect and retain your personal and health information — and the rights you hold over it under the Data Privacy Act of 2012 — is set out in our Privacy Notice, which forms part of these terms.

We aim to keep this website available, but we do not guarantee uninterrupted access. It may be unavailable during maintenance or because of circumstances beyond our control. If online booking or registration is unavailable, please call (064) 201-3344 or come to the clinic — walk-ins are always accepted at reception during clinic hours.

Nothing in these terms limits our responsibility for the clinical care we provide, or excludes any liability that cannot lawfully be excluded under Philippine law.

Subject to that, we are not liable for loss arising from your reliance on the general information on this website, from any interruption or unavailability of the website, or from any delay or failure in the delivery of an SMS or email reminder by a network or provider outside our control. Reminders are a courtesy; responsibility for attending your appointments and completing a course of treatment remains with you.

This is version 1.0, effective 1 January 2026. We may update these terms and will publish any new version on this page with a new version number and effective date. Continuing to use the website after a change means you accept the updated terms.

These terms are governed by the laws of the Republic of the Philippines, and any dispute is subject to the exclusive jurisdiction of the courts of Sultan Kudarat.
